November 12th, 2021

 

Per CMS guidelines updated 11-12-21 the facility is now fully open to unrestricted indoor and outdoor visitation. 

Although there is no limit on the number of visitors that a resident can have at one time, visits should be conducted in a manner that adheres to the core principles of COVID-19 infection prevention and does not increase risk to other residents. Visitors should not walk around the facility, rather go directly to the resident's room. 

 

Core Principles of COVID-19 Infection Prevention 

• Visitors who have a positive viral test for COVID-19, symptoms of COVID-19, or currently meet the criteria for quarantine, should not enter the facility. All visitors will be screened prior to visiting residents.

• Hand hygiene (use of alcohol-based hand rub is preferred)

 • Face covering or mask (covering mouth and nose) and physical distancing at least six feet between people, in accordance with CDC guidance.

 

We encourage visitors to be tested on their own before coming to the facility (e.g., within 2–3 days). 

CMS strongly encourages all visitors to become vaccinated and facilities should educate and also encourage visitors to become vaccinated.

August 19th, 2021

the facility has 9 residents under PUI  and was quarantined on August 17 2021 per CDC and DOH guidelines. 

The St.Louis County positivity rate continues to rise from 11% to 11.8% . This facility continues to test all  42 staff and 92 residents regardless of vaccination status, every 3 days for COVID-19. 0 positive results

July 30th, 2021

This facility has 9 residents under PUI and quarantine, July 13, 2021-July 30, 2021, per CDC and DOH guidelines.

 

The St. Louis Community positivity rate continues to rapidly increase from 9.4% on Monday, July 26, 2021, to 10.45% as of Thursday, July 29, 2021, late afternoon. Due to this rapid increase, this facility has begun testing staff members every 3 days. As of Tuesday, July 27, 2021, all staff members were tested with 0 positive results. All staff members available today, Friday, July 30, 2021, have been tested with one staff member testing positive. This staff member was sent home immediately for quarantine.

 

This facility has contacted our local DOH to notify them of our positive staff members. Per their recommendations, all residents, who may have been in contact with this staff member, have been tested with 0 positive results. We will continue to test these residents for the next 14 days every 3-7 days per the guidelines. Residents with representatives or POA’s have been notified via phone of our positive staff member and steps we are taking to ensure our residents remain safe.

July 27th, 2021

The facility has 6 residents under PUI and quarantined, July 13, 2021-July 26, 2021, per CDC and DOH guidelines.

 

The facility has 0 residents who have been tested in the past week per CDC and DOH guidelines. 40 staff members have been tested, 0 positive results.

The facility continues the recommendations of the LDOH for COVID-19 testing for residents and staff.

 

The St. Louis County community positivity rate remains increasing. It is currently at 9.4%. Due to this increase, all staff members are tested weekly and this facility will continue weekly testing on staff as per the CDC and DOH recommendations until the rate drops below 5%.

 

This facility continues monthly COVID-19 clinics to administer the vaccine to our residents and staff members. On 1/21/2021, 13 residents consented and received the vaccine, as well as 14 staff members. On 2/9/2021, 37 first doses and 27 second doses were administered. On 3/9/2021, 37 second doses and 3 first time doses were administered. On 4/28/2021, 3 second doses, 13 on-time vaccines and 3 first doses were administered. On 5/25/2021, there was a single first time dose administered and four second doses, In addition, on this same date, 7 one-time doses were administered to residents. No staff members participated in the clinic. On 6/23/2021, 1 first time dose was administered, 3 one-time doses were administered to residents and 4 staff members received the vaccine. One first time dose and 3 one-time doses were given to these staff members. Our next COVID-19 clinic is scheduled for Wednesday, July 28, 2021.

 

Consents were obtained for all residents and staff members who received the vaccine. Educations was provided per CDC guidelines/regulations to resident’s, residents’ representatives/POA, and staff. All staff members are encouraged to received the COVID-19 vaccine to assist in mitigating the exposure/spread of COVID-19.

 

Regarding visitation, we continue to follow the CDC/DOH guidelines. We offer indoor/outdoor designated areas for availability upon appointment. July 20th, 2021

 

The facility has 7 residents under PUI and quarantined, July 4, 2021-July 19, 2021, per CDC and DOH guidelines.

 

Community wide testing for COVID-19 has been completed, at this time. Guidelines per CDC and DOH recommendations have been met for resident testing. This community completed the required 14 day testing with 0 positive results. We will remain diligent and continue to test residents per CDC and DOH regulations.

 

The St. Louis County community positivity rate continues to increase to 9.1%. This rise has been noted over the last three weeks and we continue to monitor the positivity rate. Since the positivity rate has increased, all staff are tested weekly and as needed. All staff members have been tested with 0 positivity for COVID-19. Weekly testing will continue until the St. Louis community positivity rate is below 5% per CDC and DOH guidelines.

 

This facility continues monthly COVID-19 vaccine clinics to administer to our residents and staff. On 1/21/2021, 13 residents consented and received the vaccine. Included on this date were 14 staff members. On 2/9/2021, 37 first doses and 27 second doses were administered. On 3/9/2021, 37 second doses and 3 first time doses were administered. On 5/26/2021, the clinic administered a single first time dose and 4 second doses. In addition, on this same date, 7 one-time doses were administered to our residents. No staff members participated in this clinic. On 6/23/2021, 1 first time dose was administered, 3 one-time doses were administered to our residents. One this same date, four staff members received the vaccine. One first time dose was administered and 3 one-time doses were given to the remaining staff members. Our next COVID-19 clinic is scheduled for Wednesday, July 28, 2021.

The facility has 8 residents under PUI and quarantined, June 28, 2021-July 12, 2021, per CDC and DOH guidelines.

 

Community COVID-19 testing was completed on Monday, July 12, 2021 for residents with 0 positive results, as well as all staff members, due to one positive staff member. This completes are 14 day requirement per CDC and DOH guidelines for testing residents, at this time. This facility followed the recommendations of the LDOH for COVID-19 testing for residents.

 

The St. Louis County Community positivity rate continues to increase to 7.7% from 5.6% last week. Due to this increase, this facility continues weekly COVID-19 testing for all staff members with 0 positive results. We will continue weekly testing of our staff members until this rate drops below 5% per CDC and DOH guidelines.

 

This facility continues monthly COVID-19 clinics to administer the vaccine to our residents and staff members. On 1/21/2021, 13 residents consented and received the vaccine, as well as, 14 staff members. On 2/9/2021, 37 first doses and 27 second doses were administered. July 6th, 2021

 

The facility has 8 residents under PUI and quarantined, June 21, 2021-7/5/2021, per CDC and DOH guidelines.

​

Community wide COVID-19 testing was completed on Monday, June 14, 2021 for residents with 0 positive results as well as all staff members, due to one positive staff member. No further staff members tested positive upon the 14 day completion. On 6/29/2021, the facility had one agency staff member who tested positive for COVID-19 via Rapid test x 2 test kits around 2:30pm. The DOH was notified and this facility was instructed to test only the residents and staff to this unit, every 7 days x 14 days, as the probability of transmission was extremely low. Eight residents and 16 staff members were tested on this date with 0 positive results. This was done per the recommendation of our local DOH. The facility continues to follow CDC and DOH guidelines for retesting. This agency nurse remains quarantined, at home. On 7/6/2021, all residents to same unit tested with 0 positive results.

The St. Louis County Community positivity rate has increased significantly from last week to 5.6%. Due to this increase, all staff members are being tested for COVID-19 with 0 positive results. We will continue weekly testing with our staff until this rate drops below 5% per CDC and DOH guidelines.

This facility has held several COVID-19 clinics to administer the vaccine to our residents and staff. On 1/21/2021, 13 residents consented and received the vaccine, as well as 14 staff members. On 2/9/2021, 37 first doses and 27 second doses were administered. On 3/9/2021, 37 second doses and 3 first time doses were administered. On 4/28/2021, three second doses, 13 one-time vaccines and three first doses were administered. On 5/26/2021, there was a single first time dose administered, and four second doses. June 30th, 2021

This facility had one agency LPN who tested positive for COVID-19 via Rapid test x 2 test kits on 6/29/2021 around 2:30 pm. 

 

Eight residents were tested with 0 positive results. 16 staff members were tested, including nursing, therapy, housekeeping, social services, activities, BOM, Administrator, and interim DON with 0 positive results.

 Per recommendations of LDOH the facility will continue to test staff and residents assigned to this unit for 14 days per guidelines and this nurse is to be quarantined, at home, for 10 days, per CDC guidelines for Health Care staff.  Per LDOH  there is no need to test community-wide due to the circumstances. Visitation may continue according to CDC and DOH guidelines.

June 15th, 2021

 

The facility has 13 residents for PUI and quarantined, June 1, 2021-June 14, 2021, per CDC and DOH guidelines.

 

As part of our ongoing monitoring of COVID-19, due to our positive staff member on Monday, June 7, 2021, community wide testing in the facility was completed on 6/14/2021 with 0 positive results. All staff members have been swabbed, as well, with 0 positive results. We will continue one additional week of community wide testing per CDC and DOH guidelines. I have emailed our contact from our local DOH to inform her of the testing results. I am awaiting any further direction, if any.

Community wide testing will take place on Monday, June 21, 2021, to complete our 14 day incubation period.

​

The community COVID-19 positivity rate has decreased from 4.01% to 3.2% as the vaccine continues to be administered in St. Louis County.

 

As previously reported, the facility had it’s first COVID-19 vaccination clinic on 1/21/2021. 13 residents consented and received the vaccine, as well as 14 staff. The second clinic was held on 2/9/2021. 37 first doses and 27 second doses were administered. The third clinic, held at the facility, took place on 3/9/2021. 37 second doses and 3 first time doses were administered. On 4/28/2021, the clinic administered 3 second dose vaccinations, 13 one-time vaccinations and 3 first time doses. Our fifth clinic was held on May 26, 2021. There was a single first time dose of Moderna administered and 4 second doses. In addition, there were 7 doses of the Janssen, one-time vaccine administered. All doses, on this date, were administered to residents with no employees receiving the vaccine. All consents were obtained along with the new CDC guidelines for the appropriate vaccine administered.

Our next COVID-19 vaccine clinic is scheduled for June 23, 2021. Consents/Declinations will be obtained from our residents. All staff members are encouraged to receive the COVID-19 vaccine to assist in mitigating the exposure/spread of COVID-19.

On Monday, June 7, 2021, visitation was temporarily cancelled for precautionary measures. Visitation was resumed on Tuesday, June 8, 2021. The facility has 12 residents for PUI and quarantined per CDC and Department of Health Guidelines. Residents have received COVID-19 vaccination per hospital transfer records.

The facility has 0 residents who have been tested for COVID-19 per CDC guidelines.

 

The community COVID-19 positivity rate has decreased since last week from 3.7% to 3.5%, at present, for St. Louis County. This facility will continue monthly employee testing which is scheduled for the week of June 14, 2021. 

 

Community wide testing was completed on 4/25/2021 and 4/26/2021 as required by the Department of Health. The facility continues to follow all CDC/DOH guidelines to mitigate the exposure/spread of COVID-19.

The facility had it’s first COVID-19 vaccination clinic on 1/21/2021. 13 residents consented and received the vaccine, as well as 14 employees. May 18th 2021

 

The facility has 9 residents as PUI and are quarantined per CDC and Department of Health guidelines.

 

The facility has 0 residents who have been tested in the past week per recent CDC guidelines.

 

23 staff members have been tested, 0 positive.

Community Covid-19 rate has dropped to 3.4% for St. Louis County. This facility will be going to monthly employee testing.

Community wide testing was completed on 4/25/2021 and 4/26/2021 as required by the Department of Health. The facility continues to follow all CDC/DOH guidelines to mitigate the exposure/spread of COVID-19.

The facility currently has 5 residents under PUI, who are quarantined per CDC and Department of Health guidelines.

No further testing for residents, on a regular basis, is being performed per CDC guidelines.

The facility has 0 staff members for PUI.

The facility has 0 staff members who have tested positive for Covid-19.

The facility continues to follow CDC and the Department of Health guidelines to mitigate exposure/spread of Covid-19.

The facility currently has a cumulative of 6 residents as a PUI who are quarantined per CDC and Department of Health guidelines.  We continue to monitor the Covid-19 positivity rate daily at  COVID-19 Data | City of St. Louis (stlouis-mo.gov). Our area remains in the <5% range.  

 

The facility has had 0 resident who have tested positive in the past week. All residents on the unit were tested and were negative along with all employees, 0 positive.

 

The facility currently has 0 staff as a PUI in the past week.

The facility currently has a cumulative of 7 residents as a PUI who are quarantined per CDC and Department of Health guidelines.   

 

The facility has had 1 resident who has tested positive in the past week. The resident was symptomatic and sent to ER, was admitted to the hospital. Still in hospital. All residents on the unit were tested and were negative along with all employees, 0 positive.

 

The facility currently has 0 staff as a PUI in the past week. The employee is on quarantine at home.

 

 The facility currently has 0 staff who have tested positive.

 

We completed our facility-wide testing of staff on 2/18/21 and 2/22/21 as required by DOH, per Health Department recommendations we will test on the 25th and the 29th, with no positives we will continue one time per week testing of only staff. The facility continues to follow all CDC/ODH guidelines to mitigate the exposure/spread of COVID-19.

The facility has had 0 residents who have tested positive in the past week. One  Resident was symptomatic and sent to ER, was admitted to the hospital. Still in hospital. All residents on the unit were tested and were negative.

We completed our facility-wide testing of staff on 2/1/21 and 2/4/21 as required by DOH and will be completing it twice every week moving forward as required. The facility continues to follow all CDC/DOH guidelines to mitigate the exposure/spread of COVID-19.

The facility had its first COVID Vaccination Clinic on 1/12/21 at the facility for residents and staff. 13 residents consented and received the vaccine and 14 employees. The second clinic will be on 2/9/21.

The facility currently has a cumulative of 6 residents as a PUI who are quarantined per CDC and Department of Health guidelines.

The facility has had 1 resident who have tested positive in the past week. Resident is asymptomatic and is in isolation room.

The facility currently has 1 staff as a PUI in the past week. Employee is on quarantine at home and will be returning to work on Monday 1/25/21.

The facility currently had 0 staff test positive this past week.

The facility has had 1 resident who has tested positive in the past week. The resident was symptomatic and sent to the ER, was admitted to the hospital. Still in hospital. All residents on the unit were tested and were negative.

The facility had an accumulative of 61 residents who have tested positive for COVID.  Of these 61 residents who tested positive, all have completed their quarantine per CDC guidelines and are now out of isolation.  Of the 61 residents, 1 are in the hospital related to COVID. The facility had 8 deaths related to COVID.

The  26 residents that remained in the facility and were quarantined per CDC guidelines as PUI on a separate wing completed their quarantine/isolation per CDC guidelines and are now out of isolation. The facility currently has 3 readmissions who remain quarantined in private rooms according to CDC guidelines

 The facility has an accumulative of 19  staff members test positive and remain self-quarantined per CDC guidelines and 0 employees as PUI. 14 employees have returned to work per CDC guidelines.
